3) City and State of the Chapter (Most put this on the badges but not necessary) (Once again only needs to be on the first badge)

4) Also please indicate if the Badges are to be Over the pocket or Magnetic Style ) (When using please make sure the Caution emblem on the Magnet is facing where you can see the "Caution" sign, as magnet is stronger in that direction)
